Purpose: Develop centralized dashboards that provide accessible, timely, and actionable data to support community assessment, strategic planning, performance monitoring, and emergency preparedness.

Primary Objectives

  • Monitor measurable community health indicators, trends, and disparities.

  • Present an easy-to-interpret impact scorecard to support data-driven decision-making.

Rural Health-Telehealth Medicine Survey Tool Development

Project Brief: Survey Tool — Access to Care for Rural Health Providers and Populations

Purpose: Design a survey tool to assess access to health care among rural populations and rural health care providers. The instrument will collect actionable data to inform planning, resource allocation, and interventions aligned with the public health essential service domain: Access to Care.

Primary Objectives

  • Measure current access to primary, preventive, specialty, mental health, and emergency services for rural residents.

  • Identify barriers and facilitators to accessing care (geographic, financial, transportation, workforce, cultural/linguistic, technology/digital access).

  • Assess capacity, workforce challenges, service gaps, referral and coordination practices among rural providers.

  • Capture community-level needs, utilization patterns, and local perceptions of care quality and acceptability.

  • Generate data disaggregated by age, gender, race/ethnicity, income, insurance status, disability, and geographic sub-region.

Target Respondents

  • Rural residents (adults 18+; with optional modules for caregivers of children and older adults)

  • Rural health care providers and administrators (primary care clinicians, nurses, community health workers

Purpose: Develop a centralized internal tool to document and visualize departmental mission, vision, services, goals, performance measures, and quality improvement (QI) activities. The platform will support outcome tracking, strategic alignment, audits, and continuous improvement.

  • Centralize strategic statements, services, goals, objectives, and KPIs.

  • Track program outcomes, targets, trends, and progress.

  • Standardize QI plans and departmental project charters.

  • Schedule audits, document findings, and track corrective actions.

  • Use audit and performance data to support PDSA cycles.

  • Provide dashboards and reports for leadership, grants, and compliance.
